Healthcare Services, Authorized Providers collect information about you (or any Representative Dependent that may represent individuals or provide PHI for whom they have legal rights to make healthcare decisions) that you provide before or during a Pre-Consultation or Consultation. They may collect this information in different ways, including through discussions with you during the Pre-Consultation or Consultation, via text message or email with you before the Pre-Consultation or Consultation, or when you enter or upload information on our Website before the Pre-Consultation or Consultation. Authorized Providers use personal information and PHI to provide you with Healthcare Services. Authorized Providers must comply with the privacy legislation and medical record-keeping obligations to which they are subject. Authorized Providers may, but are not required to, email or text your emergency contact if they believe that you are presenting with a healthcare emergency. You are responsible for advising your emergency contact that you are providing their name and contact information to ADHD Nurse and Authorized Providers, and for obtaining any necessary consent from your emergency contact for the provision and use of that information in connection with our Website.
Third party service providers, software and services providers for a variety of services, such as technical, operational and marketing services. We require our third-party service providers to limit their use of information to what is necessary for their services. We also require them to have security measures in place that are appropriate for the protection of information. RETENTION AND DELETION OF INFORMATION
Except as otherwise permitted or required by applicable law or regulation, we will only retain your personal information for as long as necessary to fulfill the purposes we collected it for, including for the purposes of satisfying any legal, accounting, or reporting requirements. Under some circumstances we may anonymize your personal information so that it can no longer be associated with you. We reserve the right to use such anonymous and de-identified data for any legitimate business purpose without further notice to you or your consent.
After the time period during, we may securely delete the information associated with your file, except for any information that ADHD Nurse is required to retain under Applicable Law or information required by an Authorized Provider in connection with Healthcare Services. When information is deleted, it is removed from our active database. Any information that remains in our backup is securely stored and isolated from any further processing until deletion is possible.
DATA SECURITY
The security of your personal information is very important to us. We use physical, electronic, and administrative measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration, and disclosure. We store all information you provide to us behind firewalls on our secure servers.
The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our Website, you are responsible for keeping this password confidential.
